GHES Registration Requirements

For all children registering at Garden Hills:

• Child must live in Garden Hills Elementary School District
• Parent / Guardian must bring all of the following documents to the school office:
  • Child’s Original Social Security Card
  • Child’s birth certificate (original or certified copy)
  • Parent or Guardian’s proof of residence (utility bill / lease agreement)
  • Child’s Georgia Immunization Certificate, Form 3231*
  • Child’s Certificate of Eye, Ear, and Dental Examination, Form 3300*

Age Requirements:

Kindergarten
Child must be 5 years old on or before September 1, of the year they start Kindergarten

First Grade
Child must be 6 years old on or before September 1, of the year they start First Grade

Grades 2 – 5
 Parent must also bring all of the following documents to the school office:

• Name and address of last school attended
• Withdrawal form from previous school
• Latest report card from previous school showing grade placement for this school year
